What you will submit
One document (Google Doc, PDF or Word, shared with view access) containing all six parts, with the routine, the skin-check schedule and the escalation plan as tables. No real names or identifying details of any person you care for.
What to do, step by step
- Part 1 — The person and the assessment (250-350 words): describe one person you care for or realistically would (age, condition, mobility, continence, eating, memory, who else is involved). Use no real name and no identifying detail. State what they can do themselves, what they need help with, and what they must never be left to do alone.
- Part 2 — The daily routine: an hour-by-hour plan for one full day covering waking, washing, dressing, meals and fluids, toileting, activity and rest, with the person's own preferences recorded, because a routine that ignores preference is not care.
- Part 3 — Moving, handling and pressure areas: how you would move or transfer this person, what equipment or help you would use, what you would never attempt alone, and a repositioning and skin-check schedule with the specific places you would inspect and what a developing pressure sore looks like at its earliest stage.
- Part 4 — Infection control and medicines: your hand-hygiene and cleaning routine, how you would handle soiled linen and waste, and a medicines-support log showing what you would record. State explicitly what you will never do: prescribe, adjust a dose, share someone else's medicine, or give anything a prescriber has not authorised.
- Part 5 — The escalation plan: a list of specific warning signs for this person that mean stop and get help, each paired with exactly who you would call and how fast. Include at minimum breathing difficulty, chest pain, a fall with possible injury, sudden confusion, stroke signs, choking, bleeding that will not stop, a burn, and a fever in a baby. Give the real contact route you would use.
- Part 6 — Limits and sign-off (200-300 words): write honestly about what this plan does not qualify you to do and where a nurse or doctor must take over. Then have a named qualified health worker (nurse, doctor, community health extension worker or a licensed trainer) read the plan, and record their name, role, place of work and what they corrected or confirmed, with the date.

Before you submit: make your link public
If your work lives in Google Drive or Google Docs, open Share → General access and change “Restricted” to “Anyone with the link” (Viewer). Then paste the link into a private browser window: if it opens without sign-in, you are ready. A private link cannot be graded — it is the single most common reason a good project fails.
