What you will submit
One document (Google Doc with view access, or an uploaded Word or PDF file) with a headed section for each of the six parts. Tables are welcome. Everything fictional; no real personal data or employer documents.
What to do, step by step
- Choose a fictional workplace you can picture in detail: a small construction site, a block-making yard, a warehouse, a hotel kitchen, a factory floor, a filling station, a school, or an office block. Give it a name and a location. Everything in the file is fictional: do not use a real employer's documents, real people's names, or a real incident.
- Part 1. Hazard register and risk assessment: walk through your workplace in your mind (or, if you have permission, a real one, but change the names) and list at least 12 hazards across at least five hazard families. For each, record who could be harmed and how, the current controls, a likelihood and severity rating with your matrix, the additional controls you propose using the hierarchy of controls, an owner and a review date. Controls must be specific actions, not 'be careful'.
- Part 2. Job safety analysis: pick one routine task at the workplace and break it into at least six steps. For each step, list the hazards and the controls, and name who is responsible. State what permit, if any, the task would need and who signs it.
- Part 3. Emergency plan and drill record: a one-page emergency plan covering fire, a serious injury and one other emergency relevant to your workplace, with roles (who raises the alarm, who calls whom, fire wardens, first aiders, assembly point, head count), the numbers to keep by the phone, and the map or description of escape routes. Add a filled-in record of an evacuation drill you have planned (date, time to clear, what went wrong, actions).
- Part 4. Incident report and investigation: invent one incident (a near miss or a minor injury) and complete an incident report form, then a short investigation: timeline, evidence, interviews summarised, immediate causes, root causes using 5 whys or a simple cause tree, and at least three corrective actions with owners and dates. Note what statutory or client reporting would apply and say where you would confirm the current requirement.
- Part 5. Toolbox-talk plan: a four-week plan of weekly toolbox talks for the workplace (topic, why now, one key message, one question to ask the crew, how attendance is recorded), plus the full script of one talk of 5 to 10 minutes in plain language.
- Part 6. The one-page HSE plan: policy statement, organisation and responsibilities, the legal and standards register (names of the laws and standards you would check, without inventing section numbers), the training and induction plan, inspection and audit schedule, and the leading and lagging indicators you would report monthly.

Before you submit: make your link public
